1. Introduction
1.1 Purpose of the Proposal for Change
The main aim of developing this proposal is to fix a shortcoming of communicating in health facilities especially the surgical ward. They are among the leading causes of avoidable medical mistakes resulting in patient harm, longer hospitalization and raised expenses. In essence, this quality improvement work intends to minimise the frequency of these e
ors by elevating the exchanges among the personnel of the healthcare services as well as raising the safety and quality of the services provided to the patients.
1.2 Background
It is established that communication
eakdowns in care organizations are a root cause of adverse events in health care. Healthcare workers notes that inadequate communication is highly associated with medical e
ors, slow treatment, and improper patient management suggested by (Aldawood et al., 2020). This is especially so in the intensive area of operating theatres, where refined and unambiguous communication is vital and may be severely hampered through the use of interpreter. The Australian Institute of Health and Welfare (2024) states that communication failures remain a major source of adverse events which could be minimized with structured communication tools and better interdisciplinary teamwork. Solving this problem is necessary not only to improve patient conditions but also to increase coherence between employees and decrease unnecessary waste.
1.3 Objectives
· Objective 1: The communication protocol to be adopted is the SBAR protocol; apply this tool in the handover process of patient from shift to shift in the surgical ward. This tool has been created to facilitate retrieval and review of specific information with an aim of minimising on misunderstandings and to check on the consistency in conveying details.
· Objective 2: The proposed beginning quarterly interdisciplinary communication workshops for purpose of enhancing communication among members of staff focusing on matters of weakness, stress. These workshops will cover elements such as listening to a relevant patient case, speaking about a patient’s facts, and effective collaboration during handover.
1.4 Evaluation of Change
Its necessity arises from the need to determine the effectiveness of the suggested activities to enhance patient safety levels. Two specific evaluation methods will be used:
· Evaluation Method 1: An examination based on the occu
ence of incident reports concerning communication mishaps will be done before and after the implementation of the plan. In specific, the audit will include the use of erasures in the number of e
ors noted before the implementation of the SBAR tool and after. The number of people involved in communication-related accidents will also be smaller if the intervention is successful.
· Evaluation Method 2: Self-assessment questionnaires of staff intercommunication effectiveness will be conducted before and after the interdisciplinary workshops. The surveys will assess the enhancements in the quality of handovers received regarding clarity and the quality of teamwork.
