Judgments have to be made between pushing to quickly get specific outcomes and building systems and institutions. Managing the tension between saving lives and livelihoods and starting the process of re-building the state is a particular challenge in fragile states. There is no guarantee that the poor [and marginalized/vulnerable suffering from effects of growing inequities] will benefit from reforms unless they are carefully designed with this end in mind (ibid).

With globalization and liberalization it was expected that health care system will also
improve with more open access to system, however results are not pleasing as the
deficit and health care inequalities are improving day by day. Though investment in
health sector is a major contributing factor in the development of health sector and
service delivery (Wendt, 2009); however, this is not the sole contributor and now it’s
time to focus on all other parameters also, this include human resource for health care
